Sweet Mask vs. Pesky Clown is the battle between the highest-ranked A-Class hero Sweet Mask and the Mysterious Being Pesky Clown.

Inside a Ferris wheel gondola at Narinki Park, after Saitama rejects Sweet Mask's proposal to make him a superhero who can meet everyone's expectations, he leaves the gondola. Sweet Mask is left somewhat disappointed but still determined to persuade him later. Moments later, chaos erupts below, and he quickly realizes the screams come from civilians in danger.
Pesky Clown, a clown-like Mysterious Being craving attention, begins wreaking havoc across the park, having already taken down Sosshi, Sanshon, Chinpi, and Gomago, all of whom work as security guards there. As he leaps forward to crush nearby civilians with his mallet, Sweet Mask moves in to confront him.

When Pesky Clown swings his mallet toward civilians in the park, Sweet Mask intercepts it mid-motion with one hand. He declares "executing justice..." before driving the mallet back into Pesky Clown's face with the same hand, delivering a devastating and fatal blow to the monster. Pesky Clown is knocked several feet backward, his head "exploding" from the impact, but one of the three balloons he carries pops, and to Sweet Mask's surprise, the lethal wound instantly heals as the clown regains balance.

Sweet Mask suspects that Pesky Clown has merged with his clown costume and deduces that the monster can withstand two more fatal attacks, tied to the two remaining balloons. Pesky Clown then theatrically covers himself with a cloth in a magician-like act and suddenly reveals himself riding a buzzsaw unicycle. He charges forward at high speed, the spinning blades carving deep gouges into the pavement. Sweet Mask swiftly sidesteps the rush and immediately counters with a spear hand thrust, appearing to hit Pesky Clown directly in the torso. The force hurls the clown into a nearby concrete pillar, his buzzsaw unicycle embedding itself into the structure, yet it is revealed that he had blocked the strike with his mallet, surprising Sweet Mask.

Sweet Mask judges that Pesky Clown is a rapidly growing type of monster, having already managed to block one of his strikes despite starting at such a low level. Landing with his weapon and vehicle, the clown grins creepily and declares he was destined to stand in the spotlight, claiming that the more people watch him, the more power flows through him. Sweet Mask realizes that Pesky Clown is a Mysterious Being fueled by attention, capable of growing stronger the more eyes are on him. As he shouts for bystanders to evacuate, Pesky Clown and the civilians recognize from his voice that the world's most popular celebrity is standing before them.

The surrounding screams shift into cheers. Pesky Clown charges forward and slams his mallet down at Sweet Mask. Although Sweet Mask leans aside to evade, the strike clips his sunglasses, knocking it from his face, before the weapon shatters into the ground with crushing force. Two female fans rush to grab the sunglasses, and Sweet Mask shouts for them to stay back. At that same moment, Pesky Clown swings another heavy mallet-strike aimed at all three. Sweet Mask instantly pulls the women close, embracing them as he evades the blow and drives a powerful kick into the clown's left cheek, damaging his face. The clown's attack misses its intended victims and once again buries the mallet into the ground, cratering it.
Releasing the fans, Sweet Mask counterattacks. He swings his left arm across, smashing the clown in the face, then immediately follows with a straight punch to the gut and a roundhouse kick that sends him flying through the air. These successful hits cause Pesky Clown's second balloon to burst, and the monster again recovers on landing. With little pause, Sweet Mask charges and delivers a relentless flurry of punches and kicks. Every strike lands cleanly, leaving the clown unable to defend. The punishment causes his final balloon to pop, and he heals again. Meanwhile, the crowd has begun recording and livestreaming the fight, drawing even more eyes to the spectacle.

Sweet Mask presses the assault with another punch, but this time, everything changes. Pesky Clown catches his fist with his left arm, locking down on the hero's entire right arm. With monstrous strength, he crushes it inside his oversized grip. Empowered by the surge of attention, not just from the crowd but also from the rapidly spreading livestreams, Pesky Clown swells in size, transforming into a more monstrous and far more dangerous threat.

Sweet Mask once again shouts for the large crowd to evacuate, but to no avail. The clown mockingly thanks him for drawing so much public attention that he has grown so powerful. Pesky Clown boasts that he will kill Sweet Mask, then massacre the surrounding civilians and any heroes who appear, using the attention from the internet, livestreams, and more to keep growing stronger. Summoning more strength, Sweet Mask forces his arm free from the monster's hold. He leaps backward, realizing in his thoughts that he has underestimated the clown's ability. He notes that the creature has already reached disaster level Demon or higher.

Without hesitation, Pesky Clown charges forward on his unicycle. He slams his mallet down, but Sweet Mask spins his body mid-air to evade and kicks the weapon back. The mallet shatters into the ground, causing the buzzsaw unicycle to slice into it and get stuck, halting the clown's rush. Seizing the opportunity, Sweet Mask drives a left-hand chop straight into Pesky Clown's head, drawing blood as the monster spits it out. However, Pesky Clown releases the stuck mallet and quickly grabs Sweet Mask's right leg mid-air with his massive right hand. With brutal force, he swings the hero downward, smashing his entire body and face into the ground. Before Sweet Mask can recover, the clown grabs him by the hair with both hands and viciously slams his face into the pavement again and again.

The crowd watches in horror, realizing the monster is deliberately targeting the hero's face. Witnessing this demonstration of strength, Sweet Mask comes to the shocking conclusion that Pesky Clown has become a Dragon-level threat. Planting his arms firmly against the ground, Sweet Mask lashes out with a backward left kick, striking the clown's left ear and tearing it clean off. Pesky Clown screams in pain, clutching the bloody wound as he staggers back. Freed from the monster's grip, Sweet Mask begins to get up, though his face is now horribly disfigured from the repeated impacts. Before he can fully recover, Pesky Clown retaliates with a crushing punch to his torso, sending him hurtling across the park and smashing directly into the central support structure of the Ferris wheel.

As he crashes against the massive framework, Sweet Mask realizes that in his human form he cannot defeat Pesky Clown. To win, he must release his true strength by transforming into his monster form. Yet with the eyes of the public fixed on him, hesitation grips him, for revealing that side would expose his identity as a Mysterious Being. He plummets from the impact point, slamming into the ground below. The violent collision causes part of the Ferris wheel's support beam to disconnect, and the colossal structure begins to tilt and collapse. The terrified crowd screams, while two civilians trapped inside a gondola panic as the ride destabilizes. Sweet Mask begins regenerating from the severe damage to his face, and in that moment, he recalls Saitama's words: that what truly matters is what you are, not how you are seen by others. Motivated by this, he leaps toward the structure and wedges himself between the two separated flange faces. With immense strength, he holds the upper piece in place, halting the progressive collapse as his transformation into monster form begins.

Sweet Mask puts the two metal sections back together, stabilizes the Ferris wheel, and then leaps down to confront Pesky Clown again. As the dust still lingers in the air, the clown sarcastically thanks him for continuing to entertain him. He then charges towards and launches a side kick at the hero. When the dust clears, Sweet Mask is revealed to have blocked the kick using his left shoulder and right arm. He grabs the clown's right leg and tears off a chunk of flesh using his right hand, causing Pesky Clown to scream in pain. With little pause, Sweet Mask follows up with a crushing punch directly to the clown's face, sending him crashing into the ground several meters away. The punch is so powerful that most of Pesky Clown's damaged face appears to cave inward.

The transformed Sweet Mask turns to the crowd and asks if anyone was hurt, but the onlookers recoil in horror. Now that his monstrous form is fully exposed to the public, fear replaces admiration. Internally, Sweet Mask reflects that he cannot concentrate enough to revert to his human form, noting that his sense of self is fading away. With Sweet Mask's identity as a Mysterious Being revealed, public attention on the fight intensifies. News helicopters, the crowd, and internet livestreams broadcast the battle, drawing the attention of the entire world. This surge of focus fuels Pesky Clown even further. He grows two additional arms, increases greatly in size, and manifests a massive monstrous face on his stomach, filled with sharp teeth and a wide mouth. The injuries inflicted by Sweet Mask heal completely, and even his torn ear regenerates.

In this new form, Pesky Clown uses all four arms to grab Sweet Mask, each hand seizing one of his limbs, attempting to tear them off. Sweet Mask endures the pain of being pulled apart and reflects that no one will see him as a hero anymore. Despite his inner turmoil, he decides to end the fight once and for all. As Pesky Clown pulls him toward the gaping mouth on his stomach and tries to devour him whole, his massive teeth attempting to pierce Sweet Mask's body, the hero summons all his strength, and, leaping upward, he tears off the upper portion of the monster's big mouth, causing Pesky Clown to scream in agony. As he descends, Sweet Mask clasps both hands together to form a hammer strike and delivers a devastating blow to Pesky Clown's face. The impact is so immense that it crushes and scatters the monster, marking Sweet Mask's victory.

The crowd watches in horror as Sweet Mask continues to viciously scatter Pesky Clown's remains, even though the monster is already dead. Blue Fire arrives at the scene, attempting to calm the situation and asking whether it is true that Sweet Mask has been revealed as a Mysterious Being. Among the crowd, a man is upset because his ex-girlfriend is a big fan of Sweet Mask. The man throws a rock at him. At that moment, Saitama appears behind Sweet Mask and catches the rock midair. He then turns to him with an admiring look and says that "You are really cool."

In the webcomic, the security guards beaten by Pesky Clown are just unnamed guards, unlike the four specifically mentioned in the manga.
